  • For Arizona Residents:

    We currently accept Aetna, Banner|Aetna, Blue Cross Blue Shield, Carelon, Cigna, Oscar, UnitedHealthcare, and Quest.

    Please contact us to confirm that we are in-network with your specific plan and to verify your coverage and benefits.

    For Minnesota Residents:

    We currently accept Aetna, Blue Cross Blue Shield, and Cigna. We may be credentialed with additional insurance plans in the future, so if you don't see your insurance listed, please reach out to us—we’re happy to check your options.

  • If you prefer to pay privately rather than use insurance, our self-pay rates are:

    Initial appointment: $250.00
    Follow-up appointments: $125.00

    A 3% processing fee will be added to all credit card payments.

  • ADHD Assessment (QbTest): $200

    Laboratory Testing: Pricing varies depending on the type of testing and your insurance coverage.

    Pharmacogenomic Testing (GeneSight): Sliding-scale pricing based on income and insurance coverage, with a maximum out-of-pocket cost of $330.
